Ins# on each page that saves, take out date that note is printed insurance companies will think we did session then and its conflicting ( a red flag ), add location and maybe some blank spots where we can personalize a need for a note , and save it is a template, add to an existing template not creating a whole new one .. signature line, we have to sign live. Save medical number so we.dont have to enter it each time...would be happy to send you a note to see. Have to be specific on interventions used, and what type of treatment u did..i.e. CBT, DBT..etc
